Use a quieter gate

Most tour groups arrive at the main gate on Hradčany Square, where the changing of the guard draws a crowd at noon. Two calmer options:

The north gate, over the Powder Bridge, a short walk from the Pražský hrad tram stop (tram 22).
The east gate, via the Old Castle Stairs, an uphill walk from Malostranská metro. You end up at Golden Lane, so you can do the circuit in reverse while the groups are at the cathedral.

Best (and worst) times to go

Before 9:30 am: get to St Vitus right at opening, before the tour buses.
After 3 pm: a calmer second half of the day, though interiors close at 4 or 5 pm.
Worst: 10 am to 2 pm in summer, and around the noon changing of the guard at the main gate.
Season: April to October is busiest. Winter weekdays are much quieter.

Insider tips that actually save time

Do St Vitus Cathedral first. Its queue grows fastest.
Don't buy a ticket just to see the cathedral nave. The front section is free to look into.
Come back for the views at dusk. The courtyards stay open much later than the interiors, and they're nearly empty in the evening.
Travel light. Every bag is checked at the gates.
